I know that the success of Life Coaching/Stress Management/Autogenic Training mainly depends on my own commitment, my serious cooperation, my own ability and my own desire to make a change in myself. I therefore declare my willingness to actively support this safe and effective process. Although Life Coaching/Stress Management/Autogenic Training can be very effective, I know that BeWellVital Ltd. can not guarantee results of any kind.*

I understand that Life Coaching/Stress Management/Autogenic Training is not a subsitute for medical, psychological or psychiatric treatment or counseling. I acknowledge that Life Coaching/Stress Management/Autogenic Training sessions are not medical or psychological examinations or diagnostics. I understand that BeWellVital Ltd. neither issues prescriptions nor treats for any physical or mental disorders, nor does it expect or make them responsible for them.*

I agree to inform BeWellVital Ltd. in the run-up to my sessions about any physical and mental limitations that may affect my work. I exempt BeWellVital Ltd. from any liability for damages, of whatever nature, that may occur, directly or indirectly, in connection with my meetings, whether foreseeable or unforeseeable. To the same extent, I waive any claims in damages. Claims due to gross negligence remain unaffected.*

I hereby confirm that I have informed BeWellVital Ltd. in the run-up to my meetings about any physical and mental disabilities that may interfere with my work.*
